Between August 2019 and February 2020, the CDC tracked 2,807 hospitalizations and 68 deaths in the United States linked to e-cigarette or vaping product use — a cluster of severe lung injuries that came to be known as EVALI (e-cigarette or vaping product use–associated lung injury). In the Illinois–Wisconsin case series that first characterized the cluster, the median age of affected patients was 21, and about a quarter (26%) required intubation. By the time the CDC closed active surveillance in February 2020, 82% of confirmed cases involved THC-containing products, and vitamin E acetate — an oil-based thickening agent added to illicit THC vaping cartridges — was identified as the primary causative agent. Once the CDC and FDA issued warnings and states moved to remove adulterated products from circulation, the outbreak curve broke sharply.
The outbreak was genuine and serious, but it was causally specific in a way the subsequent fear has not always respected. EVALI was not a general risk of vaping; it was a risk of vaping from unregulated THC cartridges adulterated with a specific compound in the US supply chain at a specific moment. No comparable EVALI outbreak occurred in other countries, which did not have the same black-market THC vaping cartridge market. For US teens who vape regulated nicotine products — now the dominant form of teen vaping, at 7.8% of US high schoolers in 2024, down from 10.0% in 2023 — the specific EVALI risk profile from the outbreak era does not apply. The ongoing health risks of adolescent nicotine vaping are real but distinct: respiratory function effects, nicotine dependence, potential cardiovascular exposure, and the longer-term cancer question (covered in a separate entry, where the evidence supports a hazard but not yet a quantifiable lifetime probability) are documented concerns that fall outside the acute EVALI framework.
The roughest probability estimate for the outbreak period is approximately 1 in 1,070 across all US teenage and young-adult vapers, or higher still if the denominator is limited to those who used THC-containing products specifically. The current background EVALI rate for users of regulated nicotine products is an order of magnitude lower. The outbreak is the canonical example of a risk that was real, temporary, causally specific, and subsequently dissipated through intervention — yet the reputational effect on vaping broadly persists at a level uncalibrated to the current risk landscape.

Outbreak of Lung Injury Associated with the Use of E-Cigarette, or Vaping, Products (EVALI) — Final Update- Thống kê
2,807 hospitalized EVALI cases and 68 deaths reported to CDC by February 18, 2020; 82% involved THC products; vitamin E acetate identified as primary culprit- Trích đoạn
“"As of February 18, 2020, a total of 2,807 hospitalized EVALI cases or deaths have been reported to CDC from 50 states, the District of Columbia, Puerto Rico, and the U.S. Virgin Islands. Among those with information on substances used, 82 percent reported using THC-containing products. Vitamin E acetate has been strongly linked to the EVALI outbreak. CDC, states, and the FDA recommend that people not use THC-containing e-cigarette or vaping products, particularly from informal sources like friends, family, or in-person or online dealers." ”

- CDC final EVALI update — source of the 2,807 numerator only. 2,807 hospitalizations / an internal ~3 million working-estimate denominator of US teen + young adult vapers during the outbreak window ≈ 0.00094 (roughly 1 in 1,070). The denominator is not from this source; see the 2019 NYTS (Cullen et al.) entry, which anchors current youth vapers at ~5.3 million (a larger figure → rarer true rate). Since ~82% of cases involved THC products, the rate for THC-product users specifically is higher. The post-vit-E-removal era (post-2020) has a substantially lower rate; the figure here reflects the outbreak-era population-average risk across all teen/young-adult vapers.

e-Cigarette Use Among Youth in the United States, 2019- Thống kê
In 2019 (outbreak year), an estimated 4.1 million US high school students (27.5%) and 1.2 million middle school students (10.5%) currently used e-cigarettes — ~5.3 million youth vapers- Trích đoạn
“"An estimated 27.5% (95% CI, 25.3%-29.7%) of high school students and 10.5% (95% CI, 9.4%-11.8%) of middle school students reported current e-cigarette use. ... With the assumption that the prevalence estimates from this survey are nationally representative and could be used to project to national population totals for US high school and middle school students, the data would suggest that in 2019, an estimated 4.1 million high school students and 1.2 million middle school students currently use e-cigarettes." ”

- Cullen et al., JAMA 2019 (CDC-authored analysis of the 2019 National Youth Tobacco Survey, N=19,018). Establishes the outbreak-era youth vaping denominator: ~4.1 million high school + ~1.2 million middle school = ~5.3 million current youth e-cigarette users in 2019, the peak EVALI year. This is the citable anchor for the "millions of teen/young-adult vapers" denominator used in the headline. Note NYTS surveys only middle + high school students (all under 18); the EVALI case series had a median age of 21 (NEJM), so the true exposed population extends into young adults beyond this frame and is therefore ≥5.3 million. A larger true denominator makes the outbreak rate RARER than the displayed ~1 in 1,070 (which divides by an internal 3,000,000 working estimate) — the headline is thus, if anything, on the more-alarming side, not conservative.

- CDC NYTS 2024. Provides the current teen vaping prevalence denominator context (7.8% of high schoolers, ~1.21 million students). The decline from 10.0% (2023) to 7.8% (2024) — and the broader fall from the ~27% peak years earlier — confirms that the outbreak-era exposure scenario is no longer current. The current background EVALI rate among regulated nicotine vapers is substantially lower; the normalized rate reflects the outbreak-era THC-vaping scenario.
