Dipersepsikan

penduduk AS usia 12+ (pengawasan DAWN, kunjungan UGD khusus minuman energi, 2011)
Tampilkan perhitungan

US energy-drink surveillance is the only national system that codes a high-caffeine beverage as a discrete cause of harm, and it recorded about 20,783 energy-drink-related emergency department visits in 2011, of which 58% — roughly 12,054 — involved energy drinks alone. Against the ~260 million US residents age 12 and older that year, that is an annual hazard near 1 in 21,600, compounding to roughly 1 in 366 over a 59-year adult lifetime. That figure is a deliberate floor: ordinary coffee, tea, and soda overconsumption are not coded separately anywhere, so they are invisible here, and the 2011 reading predates a decade of growth in energy-drink use. For scale, an American adult is more likely to die by homicide (about 1 in 287) than to land in an ER from a high-caffeine drink under this lower-bound count — and the ER visit is rarely the dangerous part.
The interesting feature is the direction of the gap. The fear treats heavy coffee as a slow cardiac threat, but the best controlled evidence points the other way for healthy adults: the randomized CRAVE trial found caffeinated coffee did not produce significantly more premature atrial contractions than caffeine avoidance, and the Poole umbrella review reports a summary all-cause-mortality relative risk of 0.83 for coffee drinkers, with the largest reduction around three cups a day. Moderate coffee is, on balance, associated with lower risk of death and of Parkinson’s, type 2 diabetes, and liver disease. The genuinely lethal caffeine events that drive the headlines almost always involve powder or pills, where a gram-scale dose is trivially easy to swallow; the FDA puts acute toxicity near 1,200 mg taken rapidly, which is about a dozen cups of coffee in quick succession and effectively unreachable by drinking.
Where the reassurance stops is in the subgroups the average doesn’t describe. EFSA halves the safe habitual intake to 200 mg/day in pregnancy. People with existing arrhythmia or structural heart disease, and those with diagnosed anxiety disorders, get worse outcomes from doses a healthy adult tolerates. Slow CYP1A2 metabolizers carry a measurable excess: Cornelis et al. found a 1.64-fold odds of nonfatal heart attack at four or more cups a day, an effect entirely absent in fast metabolizers. And the DAWN numerator is dominated by adolescents and heavy energy-drink users consuming caffeine in concentrated, rapidly-drunk servings — a very different exposure from a mug of drip coffee sipped over an hour. The 1-in-366 floor is an average over a population that mostly drinks caffeine safely; it is not a personal verdict for anyone in those groups.

Spilling the Beans: How Much Caffeine is Too Much?- Statistik
400 mg/day not generally associated with negative effects in healthy adults; toxic effects like seizures around 1,200 mg rapidly consumed- Kutipan
“"For most adults, the FDA has cited 400 milligrams a day — that's about two to three 12-fluid-ounce cups of coffee — as an amount not generally associated with negative effects. [...] The FDA estimates toxic effects, like seizures, can be observed with rapid consumption of around 1,200 milligrams of caffeine, or less than 1/2 teaspoon of pure caffeine." ”

- Perhitungan
- The FDA's 400 mg/day threshold is the reference line for "too much" in this entry: harms cluster above it in a dose-dependent way, and the 1,200 mg acute-toxicity figure clarifies that life-threatening caffeine effects require a rapid bolus — "less than 1/2 teaspoon of pure caffeine" — which is essentially unreachable by drinking coffee (a cup carries roughly 80–100 mg; reaching 1,200 mg would mean ~12 cups in quick succession). This frames fatal caffeine toxicity as a powder/pill phenomenon, not a beverage one, and anchors the "what counts as too much" boundary used throughout the entry. Both quoted sentences were confirmed verbatim against the live FDA page (content current as of 08/28/2024) during human review.

- Perhitungan
- This is the native-rate source. The 20,783 total × 58% energy-drinks-only share = ≈12,054 ED visits where a high-caffeine beverage was the sole implicated agent (the conservative numerator — excluding the 42% of visits that also involved alcohol or other drugs). Divided by the ~260 million US residents age 12+ in 2011, this gives the ~4.6 × 10⁻⁵ annual hazard that compounds to the headline 1-in-366 lifetime floor. The doubling from 2007 to 2011 is the basis for treating the static 2011 figure as an undercount and skewing the uncertainty band upward.

Acute Effects of Coffee Consumption on Health among Ambulatory Adults (CRAVE trial)- Statistik
Caffeinated coffee not linked to significantly more premature atrial contractions (RR 1.09, 95% CI 0.98–1.20); ~50% more premature ventricular contractions and ~36 fewer minutes of sleep- Kutipan
“"The consumption of caffeinated coffee was associated with 58 daily premature atrial contractions as compared with 53 daily events on days when caffeine was avoided (rate ratio, 1.09; 95% confidence interval [CI], 0.98 to 1.20; P = 0.10). [...] The consumption of caffeinated coffee as compared with no caffeine consumption was associated with 154 and 102 daily premature ventricular contractions, respectively (rate ratio, 1.51; 95% CI, 1.18 to 1.94)." ”

- Perhitungan
- The CRAVE randomized case-crossover trial (100 adults wearing continuous ECG, accelerometer, and glucose monitors for 14 days) is the best controlled evidence on the specific cardiac fear. It found no significant increase in atrial ectopy from caffeinated coffee, a real but modest increase in ventricular ectopy, and ~36 minutes less sleep on coffee days. It does not yield a population probability — it is used to calibrate the *severity* of the modal harm (transient ectopy and sleep loss, not sustained arrhythmia) and to justify the moderate_harm / recoverable_injury framing rather than a fatal one.

- EFSA independently corroborates the FDA's 400 mg/day daily threshold and adds a 200 mg single-dose limit for healthy adults. This is a cross-check on the "what is too much" boundary rather than a probability input. EFSA's separate, lower thresholds for pregnancy (200 mg/day) are noted in the caveats as a scope limit, not converted into a multiplier.
