DAWN sampled ED visits among US residents age 12 and older; the age-12+ US population in 2011 was approximately 260 million (US Census). The annual hazard of an energy-drink-only ED visit is therefore roughly 12,054 / 260,000,000 ≈ 4.6 × 10⁻⁵ per year, or about 1 in 21,600 per year. (2) The 2011 reading is the last year of the old DAWN methodology; energy-drink consumption and the affected population have grown since. (3) Attribution requires a clinician to code the visit to energy drinks, which misses cases presenting as anxiety, palpitations, or insomnia without the drink being recorded. The true lifetime probability of any ED visit attributable to high-caffeine beverages is plausibly several times the central estimate — hence the wide band. The far more common outcomes of excess beverage caffeine (jitteriness, insomnia, transient palpitations, reflux) almost never reach an ED at all and are not counted in any surveillance system.

US energy-drink surveillance is the only national system that codes a high-caffeine beverage as a discrete cause of harm, and it recorded about 20,783 energy-drink-related emergency department visits in 2011, of which 58% — roughly 12,054 — involved energy drinks alone. Against the ~260 million US residents age 12 and older that year, that is an annual hazard near 1 in 21,600, compounding to roughly 1 in 366 over a 59-year adult lifetime. That figure is a deliberate floor: ordinary coffee, tea, and soda overconsumption are not coded separately anywhere, so they are invisible here, and the 2011 reading predates a decade of growth in energy-drink use. For scale, an American adult is more likely to die by homicide (about 1 in 287) than to land in an ER from a high-caffeine drink under this lower-bound count — and the ER visit is rarely the dangerous part.
The interesting feature is the direction of the gap. The fear treats heavy coffee as a slow cardiac threat, but the best controlled evidence points the other way for healthy adults: the randomized CRAVE trial found caffeinated coffee did not produce significantly more premature atrial contractions than caffeine avoidance, and the Poole umbrella review reports a summary all-cause-mortality relative risk of 0.83 for coffee drinkers, with the largest reduction around three cups a day. Moderate coffee is, on balance, associated with lower risk of death and of Parkinson’s, type 2 diabetes, and liver disease. The genuinely lethal caffeine events that drive the headlines almost always involve powder or pills, where a gram-scale dose is trivially easy to swallow; the FDA puts acute toxicity near 1,200 mg taken rapidly, which is about a dozen cups of coffee in quick succession and effectively unreachable by drinking.
Where the reassurance stops is in the subgroups the average doesn’t describe. EFSA halves the safe habitual intake to 200 mg/day in pregnancy. People with existing arrhythmia or structural heart disease, and those with diagnosed anxiety disorders, get worse outcomes from doses a healthy adult tolerates. Slow CYP1A2 metabolizers carry a measurable excess: Cornelis et al. found a 1.64-fold odds of nonfatal heart attack at four or more cups a day, an effect entirely absent in fast metabolizers. And the DAWN numerator is dominated by adolescents and heavy energy-drink users consuming caffeine in concentrated, rapidly-drunk servings — a very different exposure from a mug of drip coffee sipped over an hour. The 1-in-366 floor is an average over a population that mostly drinks caffeine safely; it is not a personal verdict for anyone in those groups.
