The threshold debate alone (12 ng/mL vs 20 ng/mL vs 30 ng/mL) shifts prevalence from 5% to 65%, which is less a measurement problem than a philosophical one about where “deficient” ends and “suboptimal” begins. What the data do support without much ambiguity: if you never supplement, rarely see direct sun, and live north of the 37th parallel, your serum 25(OH)D has almost certainly been below 20 ng/mL at some point. Whether that mattered is the question the field is still arguing about.
