Verzekeraars wezen in 2024 19% van de in-netwerk gezondheidsclaims af bij ACA-marktplaatsplannen, volgens KFF’s analyse van CMS-transparantierapportages. Het percentage varieerde van 3% tot 36% afhankelijk van de verzekeraar, en 37% van de buiten-netwerk claims werd afgewezen. Dit zijn geen uitzonderingsgevallen of coderingsfouten: hoewel administratieve afwijzingen (ontbrekende informatie, dubbele indieningen) een deel uitmaken, betekent het enorme volume — 8,8 miljoen afgewezen claims in één jaar — dat miljoenen verzekerden kosten dragen die ze verwachtten dat hun verzekering zou dekken. De meest opmerkelijke statistiek is niet het afwijzingspercentage zelf, maar wat er daarna gebeurt: slechts 0,2% van de afgewezen claims werd formeel aangevochten. Het systeem functioneert in de praktijk als een eenzijdige onderhandeling waarin de initiële beslissing van de verzekeraar vrijwel altijd definitief is.
Arbeidsongeschiktheidsverzekering vertelt een nog schrijnender verhaal. De Social Security Administration wees in 2024 62% van de initiële SSDI-aanvragen af, en het percentage steeg naar een afwijzingspercentage van 64% in het fiscale jaar 2025. In de heroverweging wordt 84% opnieuw afgewezen. Pas bij de hoorzitting voor een bestuursrechter — die één tot drie jaar kost om te bereiken — stijgt het goedkeuringspercentage naar 51%. Het meerfasenproces is ontworpen als filter, en het werkt: veel aanvragers geven op voordat ze een hoorzitting bereiken, en degenen die doorzetten brengen jaren door zonder arbeidsongeschiktheidsinkomen terwijl ze wachten. De financiële schade is niet de afwijzing zelf, maar het gat dat het creëert tussen het begin van de arbeidsongeschiktheid en de uiteindelijke (als die komt) goedkeuring.
Over een volwassen leven van 59 jaar nadert de kans op het meemaken van minstens één grote verzekeringsclaim-afwijzing — hier gedefinieerd als een afwijzing met meer dan $1.000 aan betwiste kosten — bijna-zekerheid voor iedereen die regelmatig een ziektekostenverzekering gebruikt. De 70% levenslange schatting is bewust conservatief, sluit kleine administratieve afwijzingen uit en houdt er rekening mee dat werkgever-gesponsorde plannen over het algemeen lagere afwijzingspercentages hebben dan marktplaatsplannen. Het risico wordt onderschat niet omdat mensen niet weten dat afwijzingen voorkomen, maar omdat de complexiteit van het systeem verzet ontmoedigt: het bezwaarpercentage van 0,2% onthult een bevolking die afwijzing grotendeels heeft geaccepteerd als een kostenpost van verzekering. Onder degenen die wel bezwaar maken, zijn de herroepingspercentages aanzienlijk hoger, wat erop wijst dat veel afwijzingen onverdedigbaar zijn op de inhoud maar winstgevend omdat ze nooit worden aangevochten.

The strikingly low appeal rate (<1%) suggests that the vast majority of denials go unchallenged, meaning the effective denial rate — denials that result in the policyholder bearing the cost — is very close to the raw denial rate. The 19% figure covers all claim types (administrative, medical necessity, prior auth); the subset of denials for medical necessity is roughly 5% of all denials.
