Боль в горле вызывается патогенами — риновирусы, аденовирусы, стрептококки группы А, вирус Эпштейна–Барр и ещё несколько инфекционных агентов ответственны практически за все случаи. От 70% до 90% острых эпизодов фарингита имеют вирусное происхождение (Bower 2012), причём риновирус является наиболее частым возбудителем. Ни один из этих патогенов не попадает в глотку через температуру напитка. Тем не менее репрезентативный в масштабах страны опрос в США показал, что 70% родителей следуют как минимум одной стратегии профилактики простуды, не основанной на научных данных (Mott 2019), а убеждения, уходящие корнями в теории о болезнях при низких температурах, входят в число наиболее распространённых народных поверий. Исследования концептуального развития подтверждают, что теории холодной погоды как причины болезней «часто упоминаются» детьми и многими взрослыми, и зависимость от них снижается лишь по мере созревания понимания теории микробов (Sigelman 2012) — и параллельное убеждение, что холодная еда вызывает инфекцию горла, опирается на тот же народно-теоретический субстрат.
Научная литература о воздействии холода и респираторных инфекциях рассказывает более сложную историю, чем просто опровержение. Эккелс (2002) предположил, что вдыхаемый холодный воздух может нарушать мукоцилиарный клиренс носа и тем самым способствовать инфекциям верхних дыхательных путей. Делает ли что-то подобное проглоченный холодный напиток, почти не изучено: у 15 здоровых испытуемых холодная вода снижала скорость движения носовой слизи с 7,3 до 4,5 мм в минуту через 30 минут (Saketkhoo et al. 1978), но инфекции не измерялись. Ирония этой темы состоит в том, что холодная еда и напитки фигурируют в клинических рекомендациях, основанных на доказательствах, как симптоматическое лечение боли в горле, а не как её причина. Мороженое на палочке, ледяная вода и мороженое снижают температуру нервных окончаний глотки и временно уменьшают болевую сигнализацию — механизм, аналогичный прикладыванию холода к растянутой лодыжке. Те же родители, которые ограничивают холодную еду для предотвращения боли в горле, часто предлагают мороженое ребёнку, у которого она уже есть, — внутреннее противоречие, подчёркивающее, как народное убеждение следует интуитивным температурным категориям, а не какой-либо связной причинно-следственной модели. Убеждение, что холодные напитки вызывают инфекцию горла, смешивает температурное ощущение с риском заражения, применяет народные категории о «холодном» и «тёплом» в организме к области, где микробиология вытеснила гуморальную теорию, и переворачивает реальную клиническую пользу холодной еды при уходе за горлом.

The statistically significant result (13 vs 5 colds, P=0.047) was interpreted by the authors as chilling unmasking latent viral infection already present in nasal passages, not as chilling causing new infection.
