認知リスク
骨髄移植を自分自身で必要とした経験のない人の多くは、登録制度が献血バンクのように機能する(行けば適合者が見つかり、移植を受けられる)と思い込んでいます。医師が12個の遺伝子マーカーのうち8〜10個を一致させる必要があるHLA適合の実態は、ほとんど理解されていません。この認識のギャップは対象によって両方向に働きます。白人患者は、登録制度が自分たちに似たドナーを中心に構築されてきたため、難しさを過小評価しがちです。一方、マイノリティコミュニティの患者は、自分の適合確率が劇的に低いことを、しばしば厳しい形で思い知らされます。2000年代初頭以降、啓発キャンペーンは改善されましたが、この構造的な不公平は一般の人々にはほとんど見えないままです。
