Pooled across 198 studies in 66 countries, the odds of a preterm birth rise about 4% for every 1C of heat exposure and 26% during a heat wave, per the 2024 Nature Medicine meta-analysis. That relative number is robust: a separate US systematic review found 90% of relevant studies reported a significant heat-preterm association. The absolute number is smaller and far more setting-dependent. Against a US all-cause preterm baseline of 10.4% per pregnancy (CDC, 2024), the largest US cohort to date (53 million births across 50 metros, JAMA Network Open 2024) found a heat wave nudged the preterm rate ratio to only about 1.02, an attributable excess near 2 per 1,000 exposed pregnancies. The global meta figure converts to roughly 24 per 1,000. The honest headline is the excess, not the baseline, and it sits near 1 in 200 with a band running an order of magnitude in either direction.
The perceived-versus-actual gap here runs in an unusual direction. Heat barely registers on the standard list of pregnancy worries, yet the relative-risk headlines, when they surface, read as alarming. Both readings miss the structure of the risk. The effect is real and replicated, but in an air-conditioned temperate setting the per-pregnancy absolute increment is small, comparable in magnitude to a lifetime drowning risk and well below the odds of having a car stolen. What makes the topic worth a number at all is that the relative effect is consistent across continents and is widening as heat waves intensify, so the population-level burden grows even where any single pregnancy’s excess stays low.
Where the headline does not apply is mostly a question of cooling and climate. The US-versus-global discrepancy tracks air-conditioning access more than biology: the JAMA cohort’s vulnerable subgroup (under 30, high-school education or less, racial or ethnic minority) carried roughly double the attributable excess, and Bekkar et al. (2020) found higher heat-related preterm risk among Black, Asian, and younger California mothers. A US systematic review summarized by the National Partnership found Black or Hispanic women with heat exposure ran double or more the preterm-or-stillbirth risk of white women. Stillbirth sits outside the headline deliberately: the global meta’s stillbirth odds ratio (1.13) has a confidence interval crossing one, and the one clearly significant mortality signal, a 34% rise in perinatal death, comes from a 16-hospital sub-Saharan African cohort rather than from cooled high-income settings. The number above is a US-anchored per-pregnancy excess, and it is least applicable to anyone who is either fully air-conditioned (lower) or living through a hot-climate heat wave without it (higher).
