サラダ用の葉物野菜、玉ねぎ、にんじんに特化して、人々が大腸菌への恐怖をどう位置づけているかを切り分けた厳密な調査は存在しません。リコールを伴うアウトブレイク(McDonald'sの玉ねぎ、Grimmwayのオーガニックにんじん)の報道は激しく、繰り返されるため、一食あたりの基礎率に比べて体感的な起こりやすさを膨らませがちです。多くの人が抱く直感は、洗った生野菜はほぼゼロリスクで、本当のSTEC(志賀毒素産生性大腸菌)ハザードは肉だ、というものですが、これは帰属データと逆です。O157による疾患は現在、牛肉よりも生鮮野菜に多く帰属されています。

The McDonald’s Quarter Pounder outbreak of October 2024 is the cleaner illustration than most people expect, because the contaminated ingredient was not the beef. CDC traced 104 E. coli O157:H7 infections across 14 states to fresh slivered onions supplied by Taylor Farms. Of the 98 patients with available information, 34 were hospitalized, four developed hemolytic uremic syndrome — the kidney-failure complication — and one older adult in Colorado died. Three weeks later a separate strain, E. coli O121, was traced to Grimmway Farms organic carrots: 48 sick across 19 states, 20 hospitalized, one HUS case, one death. Two produce items, two recalls, two deaths, inside a single autumn.
That inversion of intuition is the headline finding. The federal attribution model (IFSAC, 2023) assigns 67.9 percent of E. coli O157 illness to vegetable row crops such as leafy greens, and only 18.6 percent to beef. Combined with CDC’s 2019 burden estimate of roughly 1,730 domestically acquired O157 hospitalizations per year, that yields about 1,175 serious produce-attributable O157 infections annually. Spread across some 335 million people and a 59-year adult horizon, the lifetime probability of being hospitalized for one works out near 1 in 4,800. That sits just below the lifetime odds of dying from food poisoning of any kind and well above a plane-crash death, in a band where the risk is real but rare.
The number is built by chaining two national estimates rather than counting a single tracked event, so its uncertainty is wide, running from roughly 1 in 13,000 to 1 in 2,100. It also undercounts in one direction and overcounts in another. It undercounts because the 67.9 percent attribution is specific to O157, and the carrot outbreak’s O121 strain — like most non-O157 Shiga-toxin E. coli — falls outside it; the serious-STEC-from-produce total is therefore somewhat higher than the headline. It overcounts for the typical adult because both serious infection and HUS concentrate in the very young. In FoodNet surveillance (Gould et al.), 6.3 percent of all O157 infections progressed to HUS, but 15.3 percent did among children under five, roughly 2.4 times the average. A salad-eating adult and a toddler eating the same recalled carrots do not face the same downstream risk of kidney failure, and the per-meal base rate stays low for everyone between the recalls that briefly make it feel otherwise.
