2025年1月、Tobacco Induced Diseases に掲載された系統的レビューは、現在の証拠が許す最も明確な答えを示した。すなわち、ベイピングが体をがんに関連する損傷にさらすという実質的な証拠がある — 酸化ストレス、遺伝毒性、DNA損傷 — 一方で、ベイピングをする人に実際にがんを引き起こしたという測定された証拠はない。このレビューは、一度も喫煙したことのないベイパーに有意ながん罹患を見いださず、ヒトにおけるがん証拠全体を限定的だと記述した。この組み合わせは矛盾ではない。それはハザード(この物質はDNAを損傷しうる)とリスク(これがあなたのがんの確率である)の違いであり、ベイピングについては前者は記録されているが、後者は今のところ測定されていない。

Two structural barriers explain the gap and are unlikely to clear soon — (1) latency: solid-tumour cancers from inhaled carcinogens typically take 20–40 years, and widespread vaping is only ~10–15 years old, so no exposed cohort has aged into the onset window; (2) confounding: most vapers are current or former smokers, so the small never-smoker vaper cohort is young and produces near-zero events with uselessly wide intervals. No attributable fraction or relative risk is available to convert into a lifetime probability, so none is published here.
